Depending on the specific program, eligibility, assessment, and approved services, an individual may qualify for assistance with everyday activities, personal care, respite, nursing, or other applicable supports.

Eligibility and services are determined by the applicable Medicaid program and authorized entities—not by Just Peachy Home Care.

Depending on the Medicaid program and the individual's approved plan of care, authorized services through Just Peachy may include, but are not limited to:

Medicaid Programs We Serve

ICWP

Independent Care Waiver Program

Supports eligible adults with severe physical disabilities or traumatic brain injuries who meet applicable level-of-care requirements, providing home- and community-based services that promote independence and help individuals remain in their communities.

SOURCE & CCSP

Elderly & Disabled Waiver Program (EDWP)

Supports eligible older adults and individuals with disabilities who meet applicable level-of-care requirements, helping them receive authorized services and support while remaining in their homes and communities as an alternative to nursing-facility care.

GAPP

Georgia Pediatric Program

Provides in-home services for eligible medically fragile children with qualifying medical needs. Authorized services may include skilled nursing and personal care support to assist children and their families in the home and community.
